For Patients New To Our Office

Patient Forms

Your initial visit in our office will typically include a consultation, exam and if appropriate, your first chiropractic adjustment. We ask our patients to arrive 15 minutes before their first appointment to complete paperwork. The initial appointment usually takes 50-60 minutes, including paperwork. Follow up visits are considerably shorter once we have determined what the problems are and the appropriate treatment course.

Consultation

Next, you will have a consultation with the doctor to discuss your health-related problems, concerns, and potential treatment options. This initial visit is designed for the doctor to learn more about you, your condition, and expectations to determine how chiropractic care can meet your goals.

Examination

After your consultation, the doctor will perform a complete chiropractic examination testing your reflexes and flexibility. Other standard neurological, orthopedic, postural, and physical tests may be performed as well. However, nothing  will be done in our office  without your consent.

X-Ray Studies

After your consultation, the doctor will perform a complete chiropractic examination testing your reflexes and flexibility. Other standard neurological, orthopedic, postural, and physical tests may be performed as well. However, nothing  will be done in our office  without your consent.

Report of Findings

Once the information is collected  and examinations are performed, the doctor will give you a detailed report of all findings on the initial or second and answer any questions including:

  1. How can you help me?
  2. How often do I need to come in?
  3. What will  my treatment  cost?

After reviewing your health history,  goals, and examining your spine and X-rays, the doctor will discuss recommendations and notify you if  your condition  requires care with other providers.   The doctor will  provide the best  treatment and  wellness program for your needs.

Treatment

At the conclusion of this initial  appointment, you can choose to  experience your first treatment. This may include spinal adjustments, physiotherapies, and/or soft tissue release techniques.  Treatment is interactive, so  you can express concerns about the different styles of treatment  anytime.
